i was a 34 b pre op and now have 380 cc saline under the muscle TUBA , my right implant is sitting too high still a year later. Im getting a capsulectomy and putting in 550-600 saline since i want bigger . I'm worried i will still have a problem with my right breast? my new ps said the original pocket was probably not placed correctly since it was TUBA .. I'm having this incision through the crease. I'm worried the implants will be too big for my chest and it'll sit high again, or worse, both!
Answer: Is going bigger a mistake? 380-550 or 600 In regards to breast implant size/profile that will best help achieve your goals, I think it will be in your best interest to spend more time with your plastic surgeon. Careful measurements, dimensional planning, and careful communication will be key. Generally speaking, the best online advice I can give to ladies who are considering revisionary breast augmentation surgery (regarding breast implant size/profile selection) is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you're looking for. Ask to see lots of examples of his/her work. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining which operation and/or breast implant size/type/profile will most likely help achieve your goals.In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or "C or D cup" or "too big" etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate. The use of computer imaging may also be helpful during this communication process. Regardless of what you might hear, there is no reliable correlation between volume of breast implant used and increase in breast size achieved. 3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Try sizers at your doctor's office with an unlined bra to help you see what options you have. As for the TUBA, its a challenging procedure and very few surgeons employ it...

It is hard for any of us to comment on what size is right for you without seeing you and knowing the dimensions of your chest. If you trust your surgeon and believe in them then you should trust that they are guiding you to the right implant.

Answer: What is too big ? All of aesthetic plastic surgery is balance between body shape and body structures. The TUBA technique is great, but when there is an unsaticfactory result, an open approach is almost always necessary. To see a significant change in breast size usually means a 25-30%increase in volume from the prior implant. So....you would need an implant volume of approximately 475 cc to show a meaningful enlargement. That size may be too large. Talk to your PS for the best opinion.I wish you well !

Many surgeons use a 3-D system called VECTRA which can help you envision approximately what your results will look like with different style and size implants. I have the discussion with patients who want bigger implants that they have to accept an increased risk of trouble down the line. A 600 or more CC implant is fairly heavy. In a woman without a lot of breast tissue, it will push out on the breast tissue to thin it and pull down on your chest skin putting you at risk for the implant falling lower or stretching your skin out. In a large breasted woman to begin with, a 600 cc implant is sometimes necessary to note a significant change. As with most things, the surgery has to be built around you and your body to get your best results. If you are not sure of your decision, have the discussion with your surgeon.
