My nose is large, crooked, and harsh, particularly with harsh nostrils, and has a cleft that makes it look like a butt. I don't want an upturned nose, in fact, I would like no change to how it hangs, closeness to my lips, etc. I would just like for it to not be so fat, my nostrils to not be so harsh (probably this will be helped when I get fat transfer, already sure I need that for my empty face), and to get rid of the cleft. It would be nice if I could fix how crooked it is, but if that makes the surgery much more complicated, it's not as important. Would also like to get rid of the bump on the bridge, but, again, this isn't super important.

Based on your photos, you are a great candidate for rhinoplasty. The large curved cartilages that make up the wide tip can be made straight, which will narrow the tip and get rid of the cleft at the same time. The nose does not need to be lifted to do this. Based on your photos, you appear to have very thin nasal skin which is why we can see your right and left tip cartilages completely through the skin, and also the natural cleft that lies in between your two tip cartilages. Rhinoplasty surgery may be considered for achieving a narrower and more symmetrical nasal tip appearance, along with dorsal bump removal and nasal straightening. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Fascial grafts may also be used to help improve tip refinement. You would benefit from an open rhinoplasty with possible septoplasty. The crooked nasal bones need to be straightened and the septum is probably deviated as well.
