Im always asked, "are you okay?" Or told, "smile!" I feel I have a strong Cupid's bow, pointy at the tips of the bow and my Vermilion borders are straight down, I feel my lips make me look like I'm always upset, sad, depressed and mean. From the picture it looks hypertrophic - stretched out. It is most likely in a high tensile area and has thus stretched. If it is a little sunken then some fat can be injected beneath the skin.
