Question

Im always asked, "are you okay?" Or told, "smile!" I feel I have a strong Cupid's bow, pointy at the tips of the bow and my Vermilion borders are straight down, I feel my lips make me look like I'm always upset, sad, depressed and mean. My Vermilion borders are also uneven. What would you suggest to make me look less depressed and mean?

If anything you require just a little volume to the upper lip.

Dear lm3,I perform a lot of lip injections with fillers such as juvederm and it works great. Many times I need to put more in one side than the other to attempt to even out the vermillion border (The red line of the lip). This helps provide more symmetry at times.
Another Option: For patients looking for a more long lasting result, I recommend fat grafting. That is, using your own bodies fat to make your lips naturally look more shapely. The bonus is that the results last many many years.
In general, I avoid using permanent prosthesis because I find that the lips are such dynamic and sensitive structures, the end result is not natural.Daniel Barrett, MD, MHA, MS

Fillers are great to add volume and improve any unevenness.
I would be also interested to see if you need volume in cheek area. that would offer fresh look and lift up your lower mouth (around lips)

I would add volume to the sides to help balance the lip so the Cupid’s bow is less of a focal point for you.

Thanks for the photos and the opportunity to answer your question.  Your lips appear somewhat "pinched" even though you have a nice shape and volume.  I suggest Botox in the depressor angularis oris, the muscle that pulls the lip corners down, and volume in the chin and the lateral upper lip as well as between the lateral lip and nose.  Treating the area around the lip will make the lip a little fuller and the entire area will look softer, not pinched.  Choose an experienced cosmetic dermatologist who will look at your entire face and recommend the best solution for the area that concerns you.  All the best, Alexis Parker MD

Hi Dear
After I saw your picture even though your concern is your lips but I think if you get filler in mid face and around your more mouth will give the happy look you want, rather than filling your lips (you have a full lips already). Filler in right places will do wonders in wrong place will make it worse.
