This means, that fat is subtracted from the surrounding areas of the buttocks and is injected into the buttocks to shape and enhance them. If you are obese, it is very difficult to give you good shape in one surgery. You might need staged surgeries to achieve your goal. With a BMI of 40, you are morbidly obese which can lead to more complications and serious complications with surgery. If you come down to a BMI of 35 or less, your results will be significantly better and your surgical risks will drop significantly. You would benefit from medical supervised weight loss program prior to any surgery. Good luck.
