Replace implants into a partial subpectoral position with mesh containment, full breast lift. This will achieve your goals for a pretty, lifted breast.

At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injure during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

I have been in practice almost 30 years and revision breast surgery is a large portion of my practice. I fixed many breast similar to yours. You would need to have your implants placed under the muscle and have a revision lift-mastopexy. You should be able to have an excellent result. Best wishes to you.

You can have revision now. Your implants need to be placed behind the muscle for better upper breast fullness and less sagging, and the lift may need to be a vertical lift for the best shape.

To make an accurate assessment regarding the outcome of any plastic surgery procedure, we typically need to see a complete set of proper before and after pictures.
If you don’t have before, and after pictures, then ask your surgeon to forward the pictures they took.
If you had implants before, it would be important to know if they were above or below the pectoralis muscle.
I think I’m more formal breast lift is going to be needed.
With your current implant position, your skin envelope is holding the weight of the implants.
I suggest having multiple in person consultations with plastic surgeons in your community.
For a second opinion consultations come prepared bringing with you a complete set of proper before and after pictures and copies of your previous operative reports.
If you’ve had more than one previous breast procedure, then bring before and after pictures and reports for each of your previous procedures.
As a general statement, I think most plastic surgeons prefer placing the implants under the pectoralis muscle, and using a more formal breast lift is generally going to be more stable with better long-term results.
Converting implant position from below the muscle to above the muscle or the other way around can be fairly complex, but it sometimes indicated.
Finding the right plastic surgeon is the most important variable, but can be more difficult than most people realize.
