So far I've had two lipo procedures in my sub mental area. The second was to remove additional fat and even out the fuller appearance on my left side. I don't believe the second procedure did much in the way of correction as my left side is still fuller 2 years post op. I sent these photos to my surgeon and was told it was fibrous tissue that needs to be surgically excised. This worries me based on the lack of resolution from the second surgery and the 4 cm long scar I now have. Any advice?

Answer: Submental Area What you are seeing is the result of excessive resection of midline fat and inadequate treatment of the entire neck including the superficial/deep fat and deeper neck structures including the muscle. I would recommend an open submental incision neck lift to repair this problem. I recommend seeing a plastic surgeon who has completed at least 6 years of plastic surgery training (ideally 7 if an aesthetic fellowship was completed).

Answer: Have an ENT evaluate the submandibular gland The area you are mentioning is very close to the submandibular gland, if you have had 2 surgeries in the area and the bulge feels soft, you may need to have the gland evaluated or reduced, fibrous scar tissue is tight and feels like a hard knot. If the area is soft, I bet it's a gland issue.

Best to have in person examination with ultrasound examination ($500). I think ultrasound guided lipo with possible My Ellevate neck lift suture will correct ($8,000). Best to be seen in person.

Answer: Making an assessment To make an assessment on the outcome of a plastic surgery procedure we need to see proper before abd after pictures. If you have had two procedures then consider including before abd after pictures of both procedures. If you don’t have proper before abd after pictures then ask your surgeon to forward the pictures they took. Based on the length of your incision I question if you just had liposuction which is usually done through small openings. I suggest scheduling several in person second opinion consultations with other local board certified plastic surgeons in your community. During each consultation ask each provider to open their portfolio and show you their entire collection of before abd after pictures of previous patients who had the same procedures. An experience plastic surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. For second opinion consultations come prepared brining with you all before and after pictures abd copies of your previous operative reports.
