I feel insecure about my lips, brow bone and forehead. I'm also concerned about my undereyes, considering I'm only twenty I look so tired all of the time. I have been on HRT for over two years now and transitioning for over three. What can I do to enhance the fem and take away the masc?
Answer: Remove the masc Brow bone reduction will make the biggest difference for you. You will almost certainly need to have the bone set back (not just shaved), so find a surgeon who does this often. Cheek implants will help to add fullness to the area under your eyes, and unlike filler they will last a lifetime. Your lips have some good volume, but the distance from the top of your lips to the bottom of your nose (where you have your piercing) is longer than average. You can shorten this with a lip lift. You can always add supernatural volume to your lips as well (an increasing trend) but if you just want to look naturally full then just a lip lift is needed. I would also recommend you at least think about how your nose will look after your brow has been reduced. I don't think it jumps out as too masculine now, but once your brow and cheeks have been feminized it will look relatively more masculine. You have great skin quality, which is the hardest thing to change, and beautiful eyes, so you already have a head start. Good luck on your journey. Bryan Rolfes, MD

In my practice I like to think of facial feminization surgery for transgender women no different from aesthetic refinements in cis-gender women. At the end of the day all faces have masculine and feminine components. I feel a lot of bad decisions can be made by considering FFS a bucket list of procedures including skull alterations and hairline surgery. In my opinion these procedures considered as the mainstay of FFS should be reserved as final options after the more significant aspects with lower risks are completed. From analyzing your pictures I think volume redistribution to balance your face would be the most beneficial and simple. I use a technique that I call Aesthetic Facial Balancing to achieve feminized and lifted appearances using either off the shelf fillers or your body's own fat. Focusing on the area around your eyes, brows and high cheeks would give you the biggest bang for the smallest buck. If you are considering more surgical approaches, a rhinoplasty to shorten your nose and rotate the tip upwards would be a nice enhancement as well. I hope this helps!

Answer: Subtle changes with FFS First off, you already look amazing! You have taken excellent care of your skin and already have many feminine facial features. With that being said, I do agree that contouring your brow bone will make a big difference. Your eyebrows and hairline are in a good position, so an incision to address the bone of your forehead would be hidden at the top of your head. I think this would open up your eyes some and make you look less tired. I keep trying to look at your photos closely, and I can't really see a lot of volume loss in your tear trough (under your eyes) to make you look tired. It may actually be your upper eyelids which sit a little low. I'd really have to see which your face animated instead of a single photo to tell. Your lips look really good, so I would have see what specific things bother you about your lips to give any recommendations. I think I can see a little bit of your upper teeth in your second photo, but your upper lip could certainly be shortened more if that's what bothers you. These are all good questions to discuss with your surgeon in an in-person or Skype consultation.
