Thanks for your question and photos, Arabella. First off, your breasts are entirely normal, and pretty. So please don't be so hard on yourself. There is quite a bit of variability not only between women and their breasts, but between both breasts of the same woman. From your photos you have very symmetrical breasts and are at a great starting point if considering breast augmentation. One of the features of the breast where there is a great deal of variation is the "take off" of the breast from the chest, that is, where the breast sits on the chest. In your case, the nipple/areola tends to project outward instead of directly forward. This is simply one of the very normal way a woman's breast can appear. If one were to enhance the breast with an augmentation that would make the nipple appear as if it is more in the center of the breast instead of the side of the breast, you would have to have a fairly wide implant that would increase the curve of the breast equally on both the inner and outer aspect of the breast. If you were to emphasize cleavage, it may actually make your nipples look further off to the sides. There is no surgical procedure that would bring your breasts and nipples inward, but a properly planned and performed great augmentation can achieve a very pretty enhancement for you without the very significant scars that would accompany any procedure that moves the nipples. Without a physical examination it is difficult to recommend with accuracy which implant type or size would be best for you, but the best way to determine which breast implant size is best for you is by first finding out your specific wishes with a visual understanding of what you hope to look like. This can be accomplished either with photographs of patients whose breasts and body type look like you, or ideally with photos of patients not only whose breasts and body type visually are similar to yours, but are also a similar height, weight, and pre-operative breast size. Unfortunately the most common reason for a woman to be unhappy after her augmentation is feeling as though she is either too big or not big enough. I ask my patients to be as specific as possible about their aesthetic goals and I then advise them accordingly. I would advise you to seek the advice of a board eligible or certified plastic surgeon experienced in breast enhancement surgery and make sure that your own sense of aesthetics aligns with that of your plastic surgeon. Your idea of “natural” or “pretty” or “proportionate” might be different from your plastic surgeon’s and it is very important for you to feel confident that you are both of like minds as far as the goals.
