You may have capsular contracture. Producing some milk after a surgery is not incredibly uncommon as there is stimulation of the nerves to the nipple-areola complex from stretch. I would recommend seeing a board-certified or board-eligible plastic surgeon with aesthetic surgery training and experience to tackle this type of problem in a safe and efficient manner. It appears you have CC. One reason is that your surgeons keep using a periareolar incision which puts you at highest risk for it. Galactorrhea can occur and the connection to this phenomenon and your capsular contracture is bacteria, hence the need to address your issue with an inframammary incision.

Producing milk after having a child is common and may not stop until menopause. An examination is needed to see if you do have capsules.

Thank you for your question and posting appropriate pictures. You may very well be a good candidate for non surgical capsular contracture treatment called Aspen ultrasound. In this treatment most patients develop resolution of a grade 4 to a grade 2 contracture after 10 non surgical and non painful sessions. Please look for providers close to you and seek a consultation for this procedure. Of course surgical correction is the gold standard but this is a great option for patients that wish not to have surgery initially.

Even with the 1 nicely posted photo, you need to be seen ONLY in person for detailed breasts examination. You also may need in office ultrasound (I am now offering that service) to dx if capsule formation, the thickness of each side of a potential capsule. Teach aggressive breasts massages to help limit capsular fibrosis formation. Also recommend seeing GYN or Endocrine MD for reasons of breasts milk leakages and possible treatments. Last option is revision surgery but my guess is wait at least 6-9 months postop. Good Luck
