You have a synform upper eyelid fold (an innie). Eyelid surgeons you visit will want to remove skin, muscle, and fat. This will not help you. You have a central partial levator disinsertion of the upper eyelid that is the basis for your innie upper eyelids. You have actual ptosis. There is also a compensatory elevation of the eyebrows. You also likely have a slight disinsertion of the lateral canthal tissues. To get the most help, you need each of these issues addressed. Because you have no spare skin, very little eyelid skin should be removed. What you need is eyelid structure: Lateral canthoplasty, Crease lowering surgery, repair of the disinserted levator aponeurosis with ptosis correction, mobilization of the pre-aponeurotic fat so it contributes volume to the upper eyelid fold, and a careful anchor blepharoplasty to recreated an appropriate upper eyelid fold.
