I have a large, prominent chin and submental area. I want this reduced but am not sure if it is fat or skin causing the issue, I am a very slim build but seem to have a lot of skin/fat on my face. I am also looking to reduce/sharpen my chin itself, and reshape the bulbous shape. Which procedures would work best for this situatuon?

Answer: Chin reduction Dear Considerate42079, it is hard to tell for sure without an examination. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations. Answer: Chin and neck profile The individual shape of someone’s chin and neck or lower 1/3 of the face is made up of all the different tissue components. Bone structure contributes most. The bone structure determines how the underlying platysma muscle drapes the neck. Skin and fat contribute as well but are typically fairly consistent in most young people. Fat removal via liposuction is the easiest and typically most effective way to improve the area especially on young people. As people get increased skin laxity due to a loss of skin elasticities and sagging of the platysma muscle with age and neck lift becomes a better option for most. For those with lacking forward projection an implant or sliding genioplasty is sometimes needed to get the best improvement. Most likely in your case Liposuction is your best option. It only addresses fat and as mentioned above other tissues contribute to shape and contour of this area as well. It’s simply difficult to reshape someone’s mandible even though bone is often the primary culprit. For those who have a substantial amount of subcutaneous fat Liposuction can give it fairly dramatic results. The potential improvement is directly related to the amount of fat available to remove. I suggest scheduling a few in person consultations with local board-certified plastic surgeons. Liposuction results vary substantially based on who does the procedure. Results should substantially reduce the double chin and create a visible defined jawline all the way to the earlobe. I would skip cool sculpting or Kybella as viable options. Chin and neck liposuction can be done as an office based procedure using local anesthesia with or without mild sedation. Best, Mats Hagstrom MD
