There is literally nothing to tighten in your face/neck other than skin, esp at this age, so surgery would be a waste of money because your skin will just stretch again. Focus on skin quality/integrity and procedures that work on those.

I would recommend taking out every bit of the filler in your face with the exception of perhaps the lips. These fillers are making the cheeks and lower face heavy giving you an older look. Also you have upper eyelid ptosis with a compensatory eyebrow elevation. This makes you look older. The right upper eyelid surgery will correct the upper eyelid ptosis and preserve and enhance the upper eyelid fold while at the same time add a firm upper eyelid platform. Finding the right care cann be very challenging.

We do a procedure exactly for this. Through a small incision in the groove under your chin we go in and remove the deep fat that is typically under the muscles. This should greatly tighten your jawline. Botox to lift the brows and masseter botox to narrow the jaw can be helpful. If the latter doesn't do mutch we also surgically can shape that area.
