Is this dog ear or hypertrophic scar tissue? (Photo)

Question

Can anyone tell me if this is a dog ear or hypertrophic scar? This is after upper eyelid surgery revision where the surgeon had to remove skin as I had a raised lump that didn’t go down but seems the raised skin is still there? It’s worse when I smile as the skin folds when I smile. What is the treatment for this as I’ve already had revision surgery and it looks even worse.

Hello there Chloe,
It is a widened scar, usually occurs if the wound closure is under too much tension or if the stitches ruptured early.  I would treat you with an ablative laser series and platelet rich plasma.  Good luck and be well.

I understand it can be frustrating to have a less than ideal outcome, and not having a clear understanding of what the problem is.
My best guess is that your result is a combination of scar tissue Contractor in an area immediately adjacent To an area that has skin laxity and skin redundancy.
The skin redundancy comes from the surrounding orbicularis muscle that causes skin redundancy when contracted which forms wrinkles. this in combination with loss of skin elasticity due to sun damage in age makes it sensitive to changes in contour immediately adjacent to an area of skin removal with scar tissue contraction. I’m guessing your surgeon may have been apprehensive to extend the scar significantly further because it would leave a visible scar on the side of your face.
At this point, I recommend you give it time and recognize that further intervention will probably lengthen the scar significantly if you want to improve the contour.Equality assessment needs to take into consideration the timeframe of when surgery was done.
I would probably give this at least six months to heal up before considering a potential next step.
Sometimes revision surgery requires a bit more of a commitment in order to really fix the problem which both patients and providers can be apprehensive to do.
