Hello, I have Asian and am considering rhinoplasty, i have quite prominent dark circles under my eye which are hereditary. I have read that after bruising from rhinoplasty you can get dark circles due to something called hemosiderin under your eyes. Is this permanent and what can i do to prevent or lessen these (given my skin tone and i have dark circles already). Thank you!

Answer: You need to use cream and avoid exposure to sunlight If the doctor uses piezzo in the surgery, there is minimal or no bruising. The bruises that occur after the surgery should never be exposed to sunlight. Otherwise, there may be permanent scars. That's why you need to use creams and protect yourself from the sun's rays.

Answer: Hemosiderin after rhinoplasty: Hemosiderin after rhinoplasty: Hemosiderin deposits can occur after bruising in people with pre-existing dark circles under their eyes. Usually it eventually resolves, but can take a few months. The best treatment is prevention. So take every possible route to avoid or minimize the bruising in the first place. Your surgeon will tell you those things to do to minimize bruising before and shortly after surgery, and he will most probably utilize those modalities available to the surgeon to minimize bleeding and resultant bruising During the surgery. Some surgeons use intravenous Decadron prior to surgery, which can markedly reduce postoperative bruising. Some surgeons mix TXA with their local anesthetic to further reduce intraoperative bleeding which results in postoperative bruising. Patients are usually advised to avoid aspirin Advil Motrin and other nonsteroidal anti-inflammatories for 10 to 14 days preoperatively since these too cause bleeding and postoperative bruising. There are a number of herbal products that some patients take which can cause bleeding and should be discontinued prior to surgery. Your surgeon will probably provide you a list of those substances. Most surgeons advise their patients to sleep in an elevated position for the first week or so after surgery and to apply cold compresses to the eyes on and off during the first 48 hours. some surgeons recommend Arnica before and after surgery and bioflavonoids which are the substances in pineapple juice to further minimize bruising. Despite all these efforts, some bruising may occur and may result in hemosiderin deposits. If so time and patience are the only treatments left.

Fortunately, there are things you can control to minimize the chances of bleeding and swelling. First and foremost, would be to follow your surgeon’s specific recommendations for their postoperative protocol. You would have to avoid medications and supplements before and after surgery that can increase your chance of bleeding, such as ibuprofen, aspirin, motrin, vitamins C and E, and fish oil. You will want to avoid things that can raise your blood pressure. So, take your pain medications consistently to keep your pain under control and avoid strenuous activities. You want to decrease the swelling by keeping your head elevated after surgery by sleeping on a few pillows or in a recliner. You may consider taking Arnica or Bromelain, which can decrease the amount of swelling and bruising after the operation. With dark circles under the eyes, they can be improved with chemical peels and light or laser therapy depending on the cause.
