As you can see in the image I have dents/creases either side of my nose which I didn’t have before. How can these be addressed in my revision rhinoplasty? It’s making my tip look droopy. Would lateral crural strut grafts help address the creases? What are the disadvantages of adding lateral crural strut grafts? Why aren’t they done in primary rhinoplasties? Can this be corrected in a closed approach?

There are many reasons for this contour irregularity. The most common that I see is a disruption of the smooth transition from the nasal bones to the upper lateral cartilages in the mid-vault (the middle part of the bridge/dorsum). I would go back to your surgeon and express your concern.
