99% of people seeking chin and neck Liposuction have a small mandible. The problem may be the entire size of the mandible or its lack of projection compared to the Maxilla. This typically represents with a premature double chin, premature jowls, lack of jaw line definition, an overbite dental occlusion, and profile pictures that will show the upper lip having significantly more forward projection than the lower lip. Most people’s fat distribution is fairly consistent, and while being overweight makes the problem worse fat distribution is not the primary underlying cause for most people having a double chin, especially at a young age. Advancing the mandible is a complex operation, but it is a treatment that addresses the primary problem. Treatment options include Liposuction, and neck lift, or addressing bone structure. Augmenting the chin will not create the same impact since it is mostly the body of the mandible that determines how the platysma muscle drapes the neck. I suggest having a few in person, consultations with both for plastic surgeons and oral surgeons. Plastic surgeons tend to be more soft tissue oriented and I think that is what you experienced with a single consultation you describe. Oral surgeons on the other hand tend to be more facial skeleton oriented, intend to see things more from that perspective. Substantial number of people don’t get the diagnosis correct on this part of the face. It is not an easy anatomic Area to make accurate assessment in because many different tissue layers contribute to shape and contour. Tissue layers that affect the appearance of the contour of this part of the face includes skin, fat, platysma muscle, sub platysmal fat, submandibular glands, and bone structure. Treatment options vary widely, including appropriate recommendations based on who you ask. Expect to get different opinions from different providers. Before choosing anyone single procedure or provider confirm that that provider has a proven track record with the procedure the recommending by asking them to show you their entire collection of before and after pictures of previous patient who had similar body or facial characteristics to your own. An experienced surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. Like I mentioned, it’s not an easy area to assess accurately, and depending on the approach, different operations may be more or less successful. In other words, a very well done neck lift could give it an exceptional result, and the same could be said for jaw advancement surgery if done by a very talented, oral surgeon. Simplistic solutions tend to be insufficient, such as the use of Kybella , or Liposuction in some people.
