I’m having a 3rd revision as the shape of my breasts haven’t improved since the last one. My surgeon is talking about doing a fat transfer to improve it. there is a dent in my left breast (pics) i’ve had uplift, implants and reduction. What’s going on here? is it my posture, or my body? All his other patients have amazing results. It’s a certified plastic surgeon. cost a lot of money. i don’t know what to do.

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

Thanks for your question. It is difficult to provide advice based on these photos alone, and if you are having a 3rd implant revision then your case will be complex by its very nature. If you have a concern about contour change in a particular area of your breast then fat transfer may help with this (assuming you have adequate donor fat). Other manoeuvres that your surgeon could consider may include working on the implant capsule or downsizing your implants in order to enable adjustment of the shape of the breast over the new implants. These are decisions that you should make together with a plastic surgeon with a specialist interest in aesthetic breast surgery.

It appears that the implant is in a good position but the breast tissue and nipple have pulled up away from the midline. One cause of this is what is known as animation deformity, where the pectoral muscle has been detached and can create this sort of appearance. If the shape is exaggerated when you contract your muscle then that needs to be addressed by re-attaching the muscle. If not, then the lift may need to be revised by moving the breast tissue into better position.

this is a difficult set of circumstances and I feel sorry for your situation.   without seeing how you were before you had any breast surgery its difficult to understand what has been happening with you.  the reality is that surgery is a biological process and there are a range of unpredictable factors relating to wound and scar healing, infection, stretching of skin / breast tissue and none of these are wholly within the control of surgeons - so good results in other patients doesn't guarantee a good result in every case and people can sometimes be unlucky with post surgical healing and outcome despite perfectly competent surgery. certainly you are in a difficult place having had several ops already to the breasts.  Fat transfer can certainly be helpful to improve irregularities.

Unfortunately, your implants have dropped because of being on top of the muscle. The wrinkle/ indent that you see is most likely your old infra- mammary breast fold. A breast lift both internally and externally with potentially moving the implant to the submuscular plane would be the most definitive option for you.

This procedure is one of the most difficult procedures of plastic surgery. Following the surgery sometimes tissues can show unpredictable reactions and these problems can occur. Hopefully it is possible to correct with fat grafting. But please note that you might need multiple sessions.
