What your surgeon can not be honest about is how many patients he or she has permanently wrecked with such a surgery. I fix these type of surgeries week in an week out. I almost all of these cases, the original surgeon thought the work looked great and the patient was emotionally destroyed by the effects of surgery. Some of these unanticipated effects can only be improved by subsequent reconstructive surgery not reversed. Hyaluronic acid fillers are fussy but much safer. Your surgeon may mean well but they are likely to harm you in ways you can't even expect. I strongly recommend that you not do this to yourself.

Without photos and/or in person consultation, it would be difficult to give you good advise. In general, it would be very hard to find a reason to lift cheeks in 20 plus year old person. Small volume addition and shaping with fillers may be more applicable. It would be in your best interest to consult another board certified plastic surgeon about this issue. Good luck.

Have you considered a trial of filler to sculpt the cheekbone and see if you like it? You are in your 20's so you need to be careful prior to subjecting yourself to any midface lifting surgery, which is typically performed for facial aging. If you like the filler then you can consider fat, which is more permanent and even implants. I do perform midface lift but not transconjunctivally.

You are asking for surgery through the inside of your lids to lift the fat on the front of the cheeks. It can be done, but, fat grafting or cheek implants are likely to be better, longer lasting, and less risks approaches. This would be done in the office, for us, and done in 40 minutes.

I am not a big fan of this procedure, especially in a 20-something year old patient. With the availability of excellent long lasting volumizers, the need for this procedure, which isn't without certain risk, should be few and far between.
