I have heard that the risk of enbloc with regards to the proximity to the lungs as I have dual placement which I understand is part under the muscle, not the sam Female, 37, dense breast tissue, suffered severe BII and have developed an autoimmune condition, Hashimotos

Hello @vee1984, thank you for your question. It is important to completely remove the capsule. Regardless of whether the implants are removed en bloc or if the capsule is formed independently of the implantation technique due to the morphology of the breast and the professional findings in the surgery. This requires knowledge and dexterity. Keep in mind that an en bloc explant surgery has two different varieties: one where you remove the prosthesis along with the capsule, and most people call that an en bloc explant surgery, but we also need to state that an en bloc explant surgery is when the prosthesis is taken out and the whole capsule is removed in pieces, however the entire capsule is explanted. Dear vee1984,
if you have issues with implants and you want them out, then they should be removed together with capsules. There are several structures between implants and lungs, so they are not that close. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

"En bloc" removal is a term used only for cancer surgery and not just removal of implants and capsulectomies. However, the entire capsule, or as much as possible, can be removed with the implants no matter the plane used.
