Butt implants are typically placed deeper than the fat grafting - usually in the muscle. For safety reasons, we avoid the gluteus muscle and fascia when doing fat grafting. Therefore, these two procedures could be done at the same time as long as you do not have any sign of infection with the buttock implants. If you do not want to do it at the same time, you should wait at least 6 weeks, but most would say 6 months.

I would suggest either doing for transfer at the time of removal. Doing it at the same time offers multiple advantages. In particular the ability to graft from inside the pocket where the implant was. This allows the surgeon to graft from new angles and new directions that are ideal for fat transfer in this area. The surgeon can confirm the depth of grafting and eliminate grafting deep into the gluteus muscle minimizing the chance of a fat embolism. This will also give the surgeon the ability to wash out any fat and not leave fat in the pocket that contained the implant. Leaving fat and encapsulated pocket where the implant used to sit will lead to excessive fat necrosis and a potential infection or fluid collection.
