(Photo) I would recommend an in person consultation with a board certified plastic surgeon with physical examination. From your photos however, it looks as though your problem is isolated in the subcutaneous fat of your lower abdomen only. If this is the case, removal of these lumps using a mini abdominoplasty approach should be possible through a c section type incision. A small amount of skin can be removed if necessary to tighten up the lower abdomen. You may still have irregularities in the skin when you are done. Be sure that your surgeon and anesthesiologist are comfortable handling your insulin and blood sugar control around the time of surgery, and that you have your diabetes well under control. Elevated blood sugar postoperatively will put your at increased risk of infection.

Answer: Careful your photos are helpful,but an exam is needed. You should change injection sites and not use the lower abdomen. Hard or dead fat may not allow normal insulin absorption.Liposuction can remove some of the hard fat and make the contour better.
