Hi! I just had a downsize done at the beginning of the year but did not achieve the look I was going for. I asked for a full B but it is still a D. So id like to go under one last exchange. Can I go from a 175cc moderate profile to maybe 100-125 low profile (I HATE upper pole fullness and projection)? Is it possible without consequence? I previously had capsulorrhaphy done when I downsized to the 175cc. Will the new surgeon have to redo this capsulorrhaphy to make room for the 100-125 cc?
Answer: Explant and Bellesoma Lift If you go from 175 cc to 125 cc that is only 50 cc difference, which is half a cup size. The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery. However, removing your 175 ml implants would not take you from a D cup to a B cup. It sounds like you already have large breasts. If that is the case, maybe a mastopexy would be your answer. An in-person examination and assessment is indicated in your case. There are scars with a mastopexy, but there should be no "stitch" marks, and usually at one year the scars are there, but not conspicuous. Thank you for your question, and good luck.

Answer: Downsize If you are a D cup with 175 cc implant, I would suggest just removing the implant. Going down only 50 cc's will not decrease you down 2 cups in size, regardless of the profile used.

Answer: Breast implants There are so many choices here it is hard to know which is right for you. However, if the implants are only 175 cc, you are a D cup, and don’t like upper fullness, taking the implants out and not replacing them is also an option. 100 cc is so small that it’s hardly worth having. But it’s your choice.

But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process.
