While a larger implant may help, you might also consider a wider implant. The downside is that it will also give you more "side boob" or volume that goes beyond the chest wall. Because of your nipple position, just moving the implant more to the center could make the nipples look too far apart.
What you describe as a "ripped internal bra" is better know in medical circles as a closed capsulotomy, and involves disruption of the natural scar envelope which encases every breast implant. The tear will eventually heal, and of itself, does not necessarily require surgical revision or...
