The implant is setlling into position. With a previously poor planned incision placement, the incision will displace on to the lower pole as the implant stretches the lower pole. This may get worse as the implant exerts its effects on the lower pole over time.

Based on the picture you submitted, it does appear that the implant has descended lower than the level of the inframammary incision, which could indicate that the implant is bottoming out. Could you possibly submit additional photos to be reviewed? It would be helpful to have a picture from the frontal view with your arms at your side, and then additional lateral views from each side.

It is difficult to say for sure from the provided photo. Double bubble can occur for a number of reasons, one of the most common being when implants settle below the natural breast fold. This can be easily if patients had their implants placed through an incision at the breast fold- if implants have bottomed out, this incision will have migrated to the lower pole skin of the breast. It will also result in the nipple position appearing higher on the breast, rather than centered over the implant.
If this has occurred, a revision is needed to elevate the position of the implant and repair the breast fold. Often, an internal bra is needed to reinforce this repair. A well-trained plastic surgeon will be able to assess this with an exam and recommend a surgical plan to correct your concerns.
