23 and feeling stuck - too much projection, want a natural look but scared of revision risks. Is a lift needed? (Photos)

Hi, I’m 23 and had breast augmentation a few years ago. While my implants look “normal,” I’m not fully happy with the result. I feel they have too much projection and lack the softer, more natural lower pole shape I’d prefer. My main concern is shape rather than size - they feel quite projected and somewhat “stuck on” the chest. I would prefer a softer slope with a more defined lower pole and inframammary fold so they fall more naturally. I currently have 440cc high-profile, textured teardrop implants. My surgeon has suggested a revision to a similar size (approximately 425–450cc) using Motiva Ergonomix implants with a lower profile to create a softer look. However, he has expressed hesitation due to risks such as malposition (e.g. bottoming out), loose skin, or the possibility I may still not achieve my desired result. I’ve also been told that achieving a more natural lower pole shape in cases like mine may require a lift, which I’m unsure about due to scarring, especially at my age. I wasn’t fully aware at the time that choosing a larger, higher profile implant could make achieving a natural shape later more difficult, so I now feel quite stuck. Is it realistic to reduce projection and improve shape with implant exchange alone, or is a lift often required? Thank you
