Have had UC for 15 yrs. Stable at the moment. Would like TT after 2 babies. Hesitant bc I worry a possible J pouch surgery in the future would ruin new abdomen. I know this is prob a question for colon surgeon but does anybody know if a laparoscopic j pouch surgery (non emergent) would cut through/ruin diastasis recti repair? I know there would be scars on the belly but I’m more concerned about having the abs cut through! No current plans for surgery. I’m healthy. Just thinking ahead.
Answer: Can I get a TT and J pouch surgery? Finding a surgeon specialized in both plastic surgery and colorectal surgery would be impossible. Though you might be able to schedule both surgeries back to back by different surgeons, doing so would not be wise. Consult with a colorectal surgeon to determine if a J pouch surgery is likely. If not, schedule the TT.

Answer: Great question in planning for the future If your question is can you get an tummy tuck at the same time as a J-Pouch? Then I would think you would be hard pressed to find a plastic surgeon (and colorectal surgeon) to do that at the same time. Too many variables for complications. But if you're just planning ahead, and thinking about doing a tummy tuck now, then there shouldn't be any contraindications to do that now and then undergo a J-pouch in the future. Don't be too concerned about the diastasis repair and any subsequent surgery to enter the abdomen. If the colorectal surgeon does plan on laparoscopic, then typically the laparoscopic ports will enter through the actual rectus muscle (which won't injure it) or through midline between the rectus muscles. These port holes are small and usually only need one or two sutures to close them. If your colorectal surgeon plans on an open procedure, then after going through your abdominoplasty incision, to enter the abdomen, they would go through the midline between your rectus muscle (essentially where the diastasis repair was done), but at that time, it would likely have scar tissue; and again, wouldn't cause you any issues once this is closed up again. Something that you may also have to consider is if your colorectal surgeon will plan on a diverting temporary ileostomy which will divert stool away from your newly created J-pouch. Obviously that would leave another scar on your abdomen once this is reversed. I think you may want to talk to your colorectal surgeon and ask them about their opinion about getting a tummy tuck and any proposed incisions for a future colectomy and J-pouch.
