Extreme burning under ears - how difficult is it to remove sutures form a neck lift that was over two years ago?

I had a neckift over 2 years ago and since immediatly after surgery I have had extreme burning under ears where the sutures are. I suspected suture reaction immediatly but my board certified plastic surgeon dismissed the possiblity and subsequently dismissed me as a patient after one post op visit. I reached out to dozens of plastic surgeons who all declined to help until I finally found one who has been giving me kenalog and 5fu injections. After 2 treatments, I have had zero relief leading me to belief it is a suture reaction. How difficult is it to have the sutures removed and what type of surgeon should I be looking for? Microsurgeon, neuro surgeon or a PS with revision experience? I have tried everything including Amitriptyline and Gabapentin but nothing gives me relief. I am absolutely desperate as this is truly destroying my quality of life. TIA
