Deep Plane Neck Lift at 32 Inquiry

Question

I am 32, not overweight, but have always had "double chin" even as a child with very little body fat. I want something permanent to remove double chin, sharpen the jawline and jowls starting to form. I am wondering if deep place neck lift is appropriate. Also: I have had mixed responses during consults about also adding chin implant (not too keen on this idea, may consider permanent filler in chin at a later time). Main concern is whether deep plane neck lift is appropriate. Thanks!

Thank you for your question regarding a deep plane facelift to correct a neck deformity in a 32-year-old female patient. Thank you for your questions, history, and pictures. In analyzing your pictures, you have excess skin and fat of your neck. Also, you have laxity or looseness of the neck muscles. Additionally, you have a small chin.

For the best results, I would suggest neck surgery. Specifically, I would suggest a neck lift that involves platysma muscle plication. The platysma is a neck muscle. When this neck muscle is tightened, the results are a snatched, defined, 90-degree neckline. In addition to the muscle tightening, you need removal of the neck fat, either with direct excision or in combination with liposuction. Lastly, you need your neck skin redraped by lifting the skin up and behind the ears.

Whether you have a deep plane facelift or SMAS plication, the most important component is that you have this neck muscle tightened. Indeed, you may even consider just doing the neck lift. For the best results, I would also incorporate a chin implant at the same time. A chin implant can also help to tighten the neck a little bit. I would not suggest permanent chin fillers. This can lead to a whole host of problems as well as migration. Therefore, most plastic surgeons would agree you would benefit most from a neck lift with platysma muscle plication, removal of neck fat, redraping of neck skin, and insertion of a chin implant. Hi from Istanbul, thank you for this question. I think you are rather young to get a neck lift. You may be able to achieve a more defined jawline by losing some weight. As for a surgical solution, liposuction combined with a chin implant could be recommended to enhance your jawline and reduce the double chin. Hope this helps!

I would recommend a necklift to address the loose skin and muscle. This is the only permanent solution that is a highly predictable outcome in the proper hands. I would go see a board-certified plastic surgeon (with plastic surgery residency and aesthetic fellowship training) for a consultation.

From the photographs presented, you do have significant fat deposits, located both above, and below the platysma muscle in the neck, which will require a deep plane neck lift. This involves liposuction to remove the fat deposits above the muscle, surgical extraction of the fat below the platysma muscle, and then a platysma plasty to significantly improve the jawline. You also appear to have a recessive chin, so small chin implant may be beneficial as well.

Thank you for the photos- you don't have an excess of skin here, but do have some start of jowl formation and submental fullness. Your chin is slightly underprojected, it should just be slightly behind the the lower lip. Liposuction of the submental region plus myellevate with or without a small chin implant would be a great procedure for you.
