What can I do to fix the neck? Especially the hollow part of the neck to make it look the same on both sides?
Answer: What's the fix? Revision deep plane facelift and deep neck lift with platysmaplasty and likely submandibular gland reduction. Thank you for sharing your question and photographs. 'This requires complex revision surgery to correct. It appears that you have some persistent platysmal banding and some soft tissue/contour irregularities in the neck. What would be required to fix this is a deep plane facelift and deep neck lift. Unfortunately, This does most often require a complete revision, including the facelift. A neck revision alone is unlikely to completely fix the problems because there is so much residual redundant soft tissue and muscle in the neck, it would create a wave of soft tissue and skin as the neck muscles are tightened. Additionally, a platysmaplasty would be required to eliminate or improve the medial platysma bands. There is soft tissue irregularity in the area of the submandibular gland, which is worse on the left. This may need to be addressed with either partial or subtotal submandibular gland removal. The right side may need to be addressed, as well. Although, this amount of deficiency may require fat transfer. This requires a complex revision surgical procedure and a simple "nip tuck" wouldn't be sufficient, unfortunately. The good news is that there are many of us that perform these types of revisions (frequently) and there are good surgeons that can help. Answer: Fix the Neck I would recommend a revision face and necklift. It appears that the muscle in the neck was not adequately treated as there is still significant platysmal banding. In addition, the asymmetry in the neck needs to be addressed by contouring the deeper structures. I would highly recommend finding an aesthetic fellowship trained plastic surgeon (completed 6 years of plastic surgery training + 1 year of aesthetic fellowship training) to perform this complex revisionary case. Answer: Neck platysmaplasty Neck bands are prominent. You could benefit from a platysmaplasty. Also, need to check if the left submandibular gland is lower than normal. Sometimes after tightening face and neck the submandibular gland becomes more noticeable as it was hiding in the loose skin before surgery. Best Dr. J

Answer: Facelift Typically when a facelift is done an incision is made under the chin and the platysma bands are sewn together. In your case you still have the platysma bands showing prominently. This can be corrected with a revision done under the chin and the front of the neck
