Hi, I received my tummy tuck last August. I am happy with the tummy tuck itself however my scar is awful. It was hypertrophic so my PS had given me two rounds of steroid injections within the last few months. Unfortunately I have noticed various burst blood vessels and I now have what appears to be a dent in the right side where the steroid injections were also used. Can anybody advise me on what to do about this?

It seems like there is neo-vascularization around the scars which is very normal in a stretched area under pressure. a serial sessions of combined pulsed-dye laser and fractional carbon dioxide laser is a good option for this. That will help the vessels, the color and the hypertrophy aswell. regards

Steroid injections can be very unpredictable and can sometimes make things worse. I would recommend scar revision under local anesthesia, with a tension free closure. This has a much better chance of improving your scar, rather than further steroid injections.

In addition, as the steroid weakens the thick scar it may have a tendency to spread. Since you're not even a year out and you've had some unsatisfactory results from treatment the first thing should be to do nothing else! You can try some over-the-counter silicone tapes a there are minimal risk. In some patients it can help flatten the scar. After about a year or so from the surgery if you're still dissatisfied with the scar you can consider excision of the scar. The difference being that during that procedure the tension will be much less than from the tummy tuck and your body is likely to respond differently. As well, you have the experience of knowing scar therapies after any further surgery should be started sooner than later. Good luck.

Steroid injections are the cornerstone of improving hypertrophic scare but they can have side effects such as fat atrophy and pronounced capillaries (blood vessels). The blood vessels responds well to laser treatment that your plastic surgeon or local dermatologist may already have. The fat atrophy however may require correction. Micro fat transfer can help to restore a small amount of lost fat volume and is my preferred treatment. Scar revision is the last option because if you form hypertrophic scars you are likely to form another one which leads to a vicious circle of scar revisions and kenalog treatments.

During a scar revision where your surgeon simply cuts out the scar and closes it again, there is much less tension on the incision during healing. This is why it is usually more successful. One of the side effects of steroid injection is tissue atrophy which can cause indentations as well. You can attempt lasers for the broken blood vessels and the scar itself, but it won't help with any indentations. That is why a full scar revision may be the best bet. It can often be done under local anesthesia without going back under
