I usually recommend wearing compression for approximately 6 weeks after surgery. I feel this helps to speed up recovery, minimizes swelling, and allows the tissues to fuse back together properly. As far as the smoking, the best answer would be "never", but at the very least you should wait 6 weeks after surgery as cigarette smoke dramatically increases the risk of wound healing issues and infections.
A compression garment is required after surgery for the next few weeks. The extra pressure helps reduce swelling and discomfort. You should not remove this garment until the second or third day after surgery. After that, feel free to move the garment temporarily for showering or laundering. Ideally, you should have circumferential pressure for 4 to 6 weeks, but you may switch to an exercise compression garment when advised. It is not a good idea to seek advice from surgeons who have not operated on you. Each surgeon has his/her own protocol. You should always follow your own surgeon's protocol.
Thank you for your question and I would first follow what your surgeon has told you. I usually recommend at least a week and two to three weeks before strenuous activities. As far as smoking is concerned, I would follow your surgeons advise but it would be better if you stopped smoking completely. The vest can be removed in the first week just to shower but should be put back on as soon as possible after showering.
Every surgeon is different in regards to their post-op management. I have my patients wear compression for at least 4 weeks. I would suggest you quit smoking until your surgeon feels it is appropriate to smoke. Best Wishes Theodore T. Nyame MD
