Deciding what’s best: tummy tuck or lipo 360 plus transfer? (Photo)

Question

I had gastric bypass 12/12/17 and lost 134 pounds. I’m 26 years old. I’m just just trying to get an honest opinion without everyone throwing a tummy tuck on me because they believe I’ll have excess skin. I know 134 pounds was really a whole other human that i dropped on when it comes to weight lol. But I’m 6’0 so a lot of the weight plays in my height. I just felt like lipo 360 and a bbl would have smooth me out maybe ? But a tummy tuck will sea the deal on my frame ? I weigh 210

I could tell just from your story about the weight loss that you would be an ideal case for a tummy tuck. You could have selected liposuction if other areas as well. I’m not a fan iIf the bbl at all. A useless procedure and a waste of money in my opinion.

Seeing your pictures, you have lot of excess saggy skin in your abdomen. There is no two doubt that you need a tummy tuck along with Liposuction 360 degrees. So, just go for it. Just the Liposuction without Tummy tuck will not tighten the skin and you will be left with in fact have much more lose skin than existing, if you do just Liposuction. So, please consider a Liposuction 360 with tummy tuck. Maybe a buttock augmentation can be added, if you want a better buttock contouring. However, go for a tummy tuck for sure.

A patient may need a TT to take care of the skin and abdominal wall laxity in the front of the body.  However, some patients may have loose skin on the back, buttocks, or lateral (outer) thighs as well. This is referred to as a posterior lower body lift.  The tummy tuck and the posterior lower body lift performed at the same time is a lower body lift. Your BMI (28.5) falls within the acceptable range to have the LBL.

It is great what you have achieved by losing weight!!! After losing 134 lbs, I would begin with your breasts first.  The technique I recommend is The Bellesoma Method.  This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.  In terms of your abdomen, you will need a tummy tuck.  If you are not planning future pregnancies, I recommend abdominoplasty with mesh reinforcement.  The mesh creates a more hourglass figure by compressing the abdominal wall resulting in early satiety and helps maintain a reduced weight.  You will probably need a circumferential lift to pull up your back and buttocks.Best Wishes,Gary Horndeski, M.D.

Thank you for your question & photos!  Based on your photos, you would be a great candidate for a tummy tuck with lipo 360!  Liposuction will remove excess fat with some retraction of skin.  A tummy tuck will address excess skin & repair of muscle.  The abdominal wall can be weakened after pregnancy and (or) significant weight gain & loss.  By repairing the muscle, not only does the appearance of your abdominal area improve, but your core strength as well.  The repair of the muscle will cinch your waist like an inner corset, which will accentuate your curves.  Your expectations and health will dictate the best procedure(s) for you. I suggest a consultation with a board certified plastic surgeon to go over the options that best address your goals.  Best of luck!

based on your photos, you look like a good candidate for a TT. Full tummy tuck should tighten your abdominal muscles and remove the excess skin. Hello and thank you for your question and congratulation on your weight loss. Based on these photos, I
believe you are a better candidate for a tummy tuck. You do have significant skin excess and liposuction alone is not enough in your case. Tummy tuck can be combined with 360 liposuction and fat transfer to  buttocks. During tummy tuck,
extra skin in the lower half of your abdomen including the stretch marks will
be removed. In addition, typically the fascial laxity/separation of recti
muscles will be taken care of by muscle plication, giving you a flat
abdomen. The added liposuction of your hips/flanks will help with creating a
natural and aesthetic waistline. Incision can be placed very low to be
hidden under bikini. With proper surgical technique, belly button can be
designed to  be a true 'innie'. Recovery from tummy tuck is 1-2
weeks. Strenuous activity and lifting heavy weights should be avoided 6 weeks
to 3 months post op. I would seek consultation with a board-certified
plastic surgeon for a full evaluation and advice. Best,
