Lower facelift 2-years post- what can cause jawline indentions? (Photos)

Hello. I am a 59 year old femal who underwent a lower facelift 2 years ago. Since that time I have been trying to work with my surgeon to establish why I have indentions along my jawline when I smile. He doesn’t seem to have any explanation and I feel like he simply wants me to go away. Any ideas what can cause jawline indentions?

Sometimes this can be from poor suturing, tissue bunching and/or just bad technique, unfortunately. I would recommend seeking a second opinion for a revisionary face/necklift. These are all my own opinions based on the photos provided. I would recommend seeing an actual plastic surgeon who has great training (in a 6 year plastic surgery residency or 3 year plastic surgery fellowship) and experience in facial aesthetic surgery (usually completion of a 1 year aesthetic fellowship after the other years of actual plastic surgery training.)

Hi, unfortunately, this is what happens in half portion facelift surgeries. When the lower face lift is performed and the upper sides are not touched at all, this tension becomes buccaling. Even if there is no relaxation on the upper side, it is necessary to explore and relax the skin deeply underneath so that the skin can easily redrape. You need to have a face lift from the beginning. You do have done tethering of the skin along the jawline. That area would need to be elevated again and hopefully would end up smoother. It’s a revision that could be done under local anesthesia

There is some sort of scar band (healing band) between the skin and the underlying tissue. This would need to be lifted and redraped to correct.

Thanks for the 2 great posted photos showing exactly your issues. I have seen this complication i my short 40+years. It is usually due to excessive swelling or black & blue in area causing skin to stretch and when healed the skin attaches to the fascia/muscle this on animation the wrinkles form. Only way to improve is re opening complete face skin to release the attachments from underlying fascia/muscles, then foam dressing with minimal facial expression for 3 weeks after surgery - it is quite an ordeal but can work. Fees in $10,000 ranges.
