Under eye bag only appears when smiling - can this be due to left over filler and should I consider a blepharoplasty? (photos)

About 6 years ago I started getting under eye filler on both eyes.The last time I went which was probably 2018 the injector basically botched me on just the left eye. It was super swollen and black/blue. I looked like rocky balboa. Fast forward to today and my left undereye now has a bag present anytime I smile. I had the filler dissolved in both under eyes about a year ago so I’m wondering if there’s still filler that needs to be dissolved or if I need to resort to a blepharoplasty
