Question

a 40yoF who has pendulous breasts despite 100lb weight los,. I am interested in breast reduction and lift. I am concerned I wont have upper pole fullness nor cleavage after surgery. I am a 36 H/I and want to reduce to a D/DD. Based on my pictures, are my concerns valid? And if so, would auto augmentation be a good and laating solution? I am not interested in implants

The technique I recommend is Bellesoma. This will reduce and reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in pain relief without excessive reduction. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.

Congratulations on your weight loss! You look like an excellent candidate for a breast reduction. Given the overall size of your breast you may want to consider a free nipple graft to mitigate postoperative healing issues. In terms of your outcome, everyone will have fullness in the upper pole immediately after surgery from swelling. In most patients this fades and long-term maintenance of this usually requires an implant. That being said you have very full breasts and may have an enduring outcome that gives you the look you are after. My recommendation would be to proceed with a breast reduction which would likely be covered by your insurance and then reassess in six months or so as the scars have begun to fade and the breast have settled into more their long-term shape. At that time if upper pole fullness remains an issue, you can consider a breast implant. Please consult with a certified plastic surgeon to develop an individualized plan.
