Breast implants after explant - would the capsule need to be removed?

Hello, I recently had my under the muscle silicone implants removed with the capsule left inside. I had them for exactly 1 year. If I were to get another set of implants over the muscle this time, would the capsule need to be removed?

Not necessarily. If your implants were only in place for about a year and you did not have capsular contracture, rupture, chronic inflammation, or other complications, the capsule often does not need to be completely removed before placing new implants.

However, the decision depends on several factors:
— How thick or scarred the capsule is
— Whether the capsule is soft or contracted
— Implant position change (under the muscle to over the muscle)
— Presence of calcification or inflammation
— Your current tissue quality

When changing from under the muscle to over the muscle, surgeons will often evaluate whether part of the existing capsule should be removed, released, or modified to help create a better pocket and smoother contour.

In many cases, especially after a relatively short implant history like yours, the remaining capsule can soften and become less significant over time. But if the capsule is abnormal or contributes to deformity, tightness, or implant malposition, partial or total capsulectomy may still be recommended.
