23F, trans, 3 years on HRT, currently ~85C (EU). Consulting in Denmark for primary augmentation. Measurements: • 174 cm, 73 kg, BMI 24 • Underbust 85 / bust 98 / waist 81 / hips 98 • Shoulder circumference 111 cm • Breast base width ~15 cm per side • Intermammary distance ~5 cm • Nipple-to-nipple ~25 cm Anatomy: Wide-set breasts, broader sternum, tight inelastic medial skin, decent soft tissue, no ptosis, symmetric. Goals: Clearly fuller silhouette, stronger cleavage (especially in bras), better balance against my shoulder width. Natural but visibly augmented. Currently considering: Motiva round, 500cc, Full profile, dual-plane, inframammary. Asking: Am I a good candidate for what I’m hoping to achieve? Are my goals realistic given my anatomy? Any concerns with my tentative plan, or alternative approaches you’d suggest for someone with my measurements and priorities? Honest input welcome, including if my goals aren’t fully achievable. many women seek improved cleavage. Narrowing the cleavage plane can be done very carefully by a skilled surgeon. Moving the implants too far medially can result in symmastia or unaboob which is very difficult to correct. That being said, the right amount of pocket dissection and implant selection can create excellent cleavage.

Based on your detailed measurements and the photos provided, you are actually a very strong candidate for a primary augmentation, and your plan for Motiva Round 500cc Full Profile implants is quite logical for your frame. Because you have a broader shoulder circumference (111 cm) and a wide breast base (15 cm), a smaller implant would likely get "lost" on your chest; the 500cc volume will provide the necessary width to fill out your footprint and help balance your silhouette. However, the biggest challenge with your anatomy is the "tight, inelastic medial skin" and your 5 cm intermammary distance; while the implants will certainly create a fuller look, getting that "touching" cleavage in a natural state may be difficult because we are limited by where your own breast tissue begins and how much the skin over your sternum will stretch. To bridge that gap and achieve the "stronger cleavage" you’re looking for, I would suggest discussing medial fat grafting (lipofilling) alongside the implants; this allows us to "soften" the inner edges and bring the breasts visually closer together without over-dissecting the pocket and risking symmastia. Overall, your goals are realistic as long as you understand that the implants will follow your natural lateral orientation, but with the right volume and perhaps a little fat grafting, you can definitely achieve that "visibly augmented" yet balanced look you're after.
