Very often volume deficit in the border region of the upper cheek and the lower lid (a very significant support region for the under-eyes) is responsible for the hollowness seen under the eyes. This same region is likewise responsible for the to the upward and outward lift that not only contours a more feminine, heart-shaped face, but diminishes the smile lines. At least from the photos provided here, this seems to be the case, and if so, then the nonsurgical use of a robust HA volumizing filler in this region to reestablish the necessary support can go a long way to remedying the hollowing (and even to some extent the dark circles) and smoothing the smile lines. I gave up injected fat many years ago, since it is expensive, requires two minimally invasive procedures (one to extract the fat from the donor sites of thighs, hips, belly and the other to inject into the recipient site), the results generally don't really last much longer than those obtained from the robust HA fillers (since separated from its original blood supply, not all the fat really survives), and the fat that does survive retains the characteristics of the region from which it is taken--meaning if belly fat develops "midriff bulge" as you age, it may do so in your face, as well. You would be wise to make sure that you are consulting with an experienced, board certified cosmetic dermatologist and of course, before agreeing to proceed, make sure to ask to see his/her personal before and after photos.
