I had gastric bypass Highest weight 355 Current weight 148 5ft 5.5in I know I want an arm lift, breast augmentation, and extended tummy tuck to remove the excess skin. Do these seem appropriate? I also want to be curvier. I have quite broad shoulders and much narrower hips and I would like to be less rectangular. How would I achieve that surgically? Is there a "best" way to combine these surgeries for best results?

You are definitely an excellent candidate for those procedures, the only surgery I would recommend best as opposed to an extended tummy tuck would be a circular tummy tuck, or also known as a belt lipectomy, this way will be able to remove all the skin from your lower abdomen and your lower back and avoid dogears. Those combination of procedures are very common. I do believe them together will give you a great outcome!

In my experience, my patients who have lost greater than 200 pounds have circumferential excess skin and have achieved a much better result with a lower body lift than with an extended abdominoplasty. The incision for a lower body lift is only slightly longer, but allows you a much more powerful correction of the entire trunk as well as the buttocks and lateral thighs which is not possible with an extended abdominoplasty. Removal of the excess tissues of your flanks and lower back with a lower body lift, will narrow your waist, and should help you achieve a more curvy figure. The brachioplasty and breast augmentation could be done at the same time, so that you have a single operation with a single recovery. As safety is a priority, you may try to find a plastic surgeon who could operate with a colleague to minimize your time under anesthesia. Best wishes.
