I am considering scheduling a consultation with a surgeon regarding a breast lift. All of my breast tissue hangs below the nipple with no upper volume. I had an augmentation about 15 years ago (saline under muscle) and subsequent removal of implants 5 years ago because I went from having saggy breasts to having giant saggy breasts. I am curious what type of lift would work for me. I’m not looking for a globe-shape, just a natural, perkier Breast.
Answer: Breast lift Dear Wrightsville,my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.Daniel Barrett, MD Certified, American Board of Plastic Surgery Member, American Society of Plastic Surgery Member, American Society of Aesthetic Plastic Surgery

Answer: Lift You would do well with the vertical lift or even the anchor lift. The doughnut lift is not an option for you and will not give a pleasing shape.

Answer: Upper pedicle mastopexy With your particular breast shape, you are more bottom heavy than top heavy. Consequently since the bottom of your breast is the heaviest and the droopiest, find a surgeon that will not keep that part. It’s the worst part why use it!?For this reason I am a huge fan of the upper pedicle Mastopexies (superior pedicle, superior medial, superior lateral). And this is also why I am not a fan of a lollipop lift because it leaves too much tissue on each side of that vertical scar, and has a tendency to drop postoperatively on each side of the scar. Remember, it is an implant that gives you the most upper pole fullness, so you will be a bit perkier but not much roundness in the upper part of the breast.

Answer: Most appropriate breast lift option for me? Great question! From the photo that you provided, it does appear that you would benefit from breast revision surgery. While you have a nice amount of volume to the lower pole, you have a moderate degree of droop and upper pole volume loss. To address this, and assuming that you are not interested in replacing your implants I would recommend the following:Formal breast lift (to help lift and shape the breasts to create a more youthful appearance)Fat Grafting (to replace lost volume to the upper pole and help create a more optimal overall appearance)Make sure you consult with a Board Certified Plastic Surgeon who specializes in breast enhancement procedures. I hope that helps and wish you the very best!Warmest regards,-Gregory A Buford MD FACS PCEOBoard Certified Plastic Surgeon and Author
