Hello! I have slight snoopy deformity in my left breast and both nipples sit low. My surgeon said he would loosen the pockets to create more lower pole, and do a crescent lift on my nipples, but I wonder if this is enough? Are there any downsides to crescent lifts? I am planning on having this procedure 10-11 months after my initial BA. 375 mod natrelle soft touch UTM 13 BWD Could there be a better option for me?
Answer: Improving results It appears your implants are riding higher than optimal, and an operation to lower the pockets would definitely improve lower pole fullness and apparent nipple-areolar complex position on the mound. However, what won't be helpful is a crescent lift. In fact, the crescent lift has no good indication. Removing a piece of skin above the nipple-areolar complex will not lengthen the distance from your nipple to inframammary fold. Only a full mastopexy or breast lift can do that, but you are not a good candidate for that because it appears you don't have skin excess, something that is necessary to move the nipple. So, please do not get a crescent lift. You should get a few second opinions from other, reputable surgeons before proceeding with your surgeon.

Answer: Considering options From the photos it appears the implants are being held too high. Your outcome can be improved through surgically helping the implant drop into the pocket. A crescent lift may not be needed. Schedule a few consultations with board certified plastic surgeons near you to better evaluate options.

Answer: Implants Usually, just lowering the inferior and sometimes lateral pockets works well. I am not a fan of the crescent lift since it tends to elongate the areola. Rather, the peri-areolar lift is better if needed.

Answer: Mini lift with smaller implants You do not want to loosen the pocket. If you loosen the pocket, the breasts will fall down and can create a double bubble and further descent of the breast implants. You can see your breasts are wider than the chest wall as your implants are too large. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, smaller implants can be placed.
