breast revision surgery is the most difficult plastic surgery procedure performed. Your lift was inadequate, your nipple-areola complex is not at the level of your implants and your implants are quite high and too large for your frame. I would recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

Your profile of your implants is too high. I would suggest going down to moderate plus, lowering the inferior pocket if needed, and perhaps tightening the skin again if needed. Before doing surgery, consider oral Accolate and external ultrasound treatments to see if the implants can drop more.
