Bottoming out? My incisions are not in the crease (Photo)

It looks like you are healing well and not bottoming out. Sometimes during an augmentation, upper abdominal skin is recruited to accommodate the increase in volume. So, even though the incision was placed at the crease, the inferior aspect of the implant may be a little lower than that as you heal from the procedure. Good luck!

You dont say if the incisions are in the orginal location, or if they have migrated up the breasts. From your photos, one cannot determine if " bottoming out" has occured. This occurs when the inframammary fold is disrupted, allowing the implant to migrate down . This can be resolved with corrective surgery. Have your surgeon evaluate you to make this determination.

Bottoming out usually happens over a long period of time. You didn't indicate how long your surgery was, but based on the healing of the scar, I'm assuming your surgery wasn't that long ago. Where the incisions are placed is usually surgeon-dependent. And having higher incisions on the breast doesn't necessarily indicate that the breasts are bottoming out or falling below the incision line. 

From the photos you provided, it doesn't look as if you are bottoming out but I am surprised about the length of the incisions given the size of your implants.  I would discuss this with your Plastic Surgeon and see what their thoughts are at this point.I hope that helps and wish you the very best!-Gregory A Buford MD FACSBoard Certified Plastic Surgeon

 One of the signs of bottoming out can be scars which start to ride up high on the breast but just because your scars are not in the crease does not mean you’re bottoming out! It really depends on what you look like straight on and how much of your implant is below the nipple versus above. From the pictures it’s just hard to tell. Another  clue is if your nipple or areola  start peeking out of the top of your bra! 

Over time, the incision that is placed just in the crease will tend to rid up a bit as the implant is settling and the skin of the breast and abdomen is accommodating the implant.  It is really not possible to tell you if you have bottomed out or are bottoming out just from the photos you have shown.Bottoming out is better assessed with frontal photos so we can see the relationship of the implant to the crease to the location of the nipple.Hope that helps.

Dear Emibee123,bottoming out occurs when the implant falls below the current breast crease or inframammary line. It's more likely to happen with inframammary incisions, which is why I try not to use the inframammary approach to any breast surgery.
Bottoming out can also occur if an implant is too large for the breast pocket. That is why it is important to choose an implant size that's correct for your body. I perform numerous measurements in the office to determine optimal implant size for my patient's bodies.

Your incisions are longer than usual and are slightly higher than usual. But it does not appear to be a result of "bottoming out". Many surgeons make their surgical  incision in the breast crease. If they do, the resultant scar is almost always on the inferior part of the breast instead of within the crease because the added volume of the implant recruits tissue from the upper abdomen and the incision in effect rises upward. In order to have a scar that predictably ends up within the crease, the initial incision needs to be made below the crease before the implant is placed. With all that said, your scar will probably end up healing very well and will not likely be a big issue once things are healed. 

should heal just above the folds as they look in your case.  Incisions at the fold are often irritated from bfra use down the road.  If the distance between the fold and incision is getting longer, then bottoming out could be occurring... but if not, you are doing great.  So focus on healing, keeping your upper poles level, and following your surgeon's instructions and you can follow yourself by measuring the distance between the fold and scar.

It does not look like you are bottoming out and the scars look well healed. The incisions are a little longer than average but a longer incision does mean that the implants have not been squeezed into the pocket.
