Unfortunately there is no good way at reducing the size of these mounds, but you may be able to make them less noticeable with dermal fillers applied to the less prominent areas of your chin and jawline around the mounds.
You may want to consider the use of volume (e.g. dermal fillers or fat grafting) to help smooth the transition between the "perioral mounds" and the depression just underneath it. I would recommend using a hyaluronic acid based filler to help smooth this transition.

The area that you point to has prominence not because of fat. This is a contour situation resulting from the musculoskeletal anatomy. Fillers might be a way of camouflaging the area if you dislike it. Depending on the muscle situation Botox might play a role as well.

My guess is that you had laser or ultrasonically assisted liposuction. This can create thermal damage to the skin and resulted hyperpigmentation. Occasionally this is permanent.
