Deep nasolabial folds, "saggy" cheeks... malar fat pad partial lipectomy? (Photo)

I am 29 years old with low body fat, athletic. I would like to have thin (lower) cheeks with less prominent nasolabial folds which cause my face to look "droopy/saggy". I have tried fillers but now looking for something else rather than adding more volume to the area. The most dramatic results would probably be mid-face lift (guessing). Is there anything in between such as lipectomy of lower part of the MALAR fat pad...? Any advice would be greatly appreciated. All suggestions are welcome as well.
