From my front and side profile I have a weak chin and the crease under my lip is super deep. What procedures would be best to enhance the jaw and fix this. A reduction with an implant? A genioplasty etc.
Answer: Reducing Lip Crease and Improving Profile I agree that your chin aesthetics can be improved. Your real issue involves your lower jaw and upper jaw. You have a short face vertically. Improvement is best accomplish with a two jaw osteotomy surgery that gives you length to your vertical dimension and advances, your lower jaw. If you decide to have a less aggressive surgical procedure, vertical increase in the chin can be accomplished with a sliding genioplasty and bone graft. The chin can be either advanced forward or slightly backwards, the space created between chin and the native lower jaw is then grafted with bone graft to increase your vertical height. It is mandatory that you have an in person consultation.

Answer: Sliding genioplasty You will need a sliding genioplasty to significantly lengthen your chin to diminish the deep mentolabial fold. One of the advantages of sliding genioplasty is that you can make changes to the length, width, or projection at the same time. The surgery is performed in the office under sedation anesthesia.

Answer: Vertical chin augmentation It appears that you are lacking vertical length and for that reason genioplasty may be better procedure than a vertical augmentation with implant. In person consultation with a surgeon familiar with both procedures would help.

Answer: Complex Mandible issues It appears that you have both deficiency and excess on different parts of your mandible. I’m guessing that the body of your mandible (segment behind your teeth) is deficient in projection and size, while the mentalis (chin) has excessive forward projection. I suggest consulting with oral surgeons in your community who have extensive experience with cranial facial assessment and mandibular surgery. I think you’ll find those for the oral surgery background to have an upper edge in comparison to those with plastic surgery training unless the plastic surgeon has completed craniofacial fellowship training with lots of hands-on experience. I don’t think you’re going to find simplistic procedures, like implants or fillers to give you the outcome you’re hoping for. Whether surgery is indicated or desirable decision is something quite personal. Any successful surgical outcome always begins with an accurate assessment and a good solid, working diagnosis. That alone, requires a much more in-depth assessment with a physical examination and possible radiological evaluation. This type of work is a bit outside of what I do on a regular basis. I’m reluctant to give you any recommendations, but would steer you towards providers with an oral surgery background as the most probable best providers for equality assessment. Good luck, Mats Hagstrom, MD
