I am a 24 year old female. I am not overweight, have been a normal, healthy weight my whole life. I have a soft jawline due to my genetics. I wouldn’t say I have a double chin, as my jaw does protrude nicely from my neck. However, I have a fat pad that sits under my chin and softens my jawline. I would love the fat pad to be removed/significantly reduced. What would be the best course of treatment for this? Photos are attached for reference. Thank you!

Answer: Custom jaw implants The definition of the entire jawline can be improved with a custom jaw implant. Another option would be jawline and submental liposuction with custom jaw angle implants. It is usually both a soft tissue problem and bony problem.

Answer: Assessing the mandible Making a mandibular assessment it’s a little bit more complicated than most people realize. You’re not alone. Most plastic surgeons aren’t good at it either. The mandible is made up of different segments. The chin or mentalis region is one of them. It’s possible to have a recessed mandible the Lacs projection in the body of them in the bowl while at the same time have a projecting mentalis or chin. That is exactly what you have. Part of your mandible is too small and part of it is projecting. The reason for your lack of jaw line definition it’s related to where the platysma muscle comes off the mandible which is in fact not projected enough. At the same time the distal part of your mandible or the chin pass projection. The reason I know this is that I can see the relationship between your upper and lower lips. Your upper lip has more for projection than your lower lip. The lip position is directly related to your dentition and you have an overbite occlusion. an overbite occlusion happens when maxillary projection is greater than mandibular projection. The perfect treatment would be a jaw advancement. That procedure is more than most people are willing to undergo. It is however based on making an accurate assessment and treating the primary problem. You don’t have an abnormal fat distribution. Fat and skin are not the primary ideologies. It’s more related to the platysma muscle whose contour is directly related to the mandible. Liposuction can give you very good improvements for individuals who have enough fat to work with and have good skin elasticity without skin laxity. Recognize that Liposuction is not treating the primary problem and sell them parent possible outcome is still going to be limited. For some individuals liposuction of the chin and neck give some results that are beyond my expectations and can be stunning. The ability to get a quality outcome from liposuction when the underlying problem is based on bone structure varies from patient to patient and to some degree who does the procedure. Generally speaking chubby young people get the best results. If the bone structure is significantly out of balance then soft tissue meant relation loses its effectiveness significantly. To get an accurate assessment in regards to bone structure considering consulting with oral surgeons. For soft tissue related surgical treatment consider consulting with plastic surgeons. Consulting with providers from both specialties will probably leave you much better informed of all treatment options at which point you can make an intelligent decision for yourself what’s the right treatment option. Understanding with the primary ideology is really helps patients except results that are less than what they had hoped for. That is not to say you can’t have a good outcome. An examination is probably in order and making an accurate quality assessment really requires an in person consultation. Oral surgeons often rely on x-rays or CT scans as well. Best, Mats Hagstrom MD
