You should find a surgeon who performs this procedure on a regular basis. Based on your information, there are a few options and concerns. If the implant is satisfactory except for the unevenness, then you can simply have the implant re-positioned and secured with small bone screws without having it removed. If you consider having it removed, there is a risk that the soft tissue of the chin will sag and look un-aesthetic. The implant has been there for 10 years so that is a concern, In some cases, I have removed the chin implant and performed a genioplasty to prevent the chin from sagging.

In the best case scenario it should be a surgeon who placed the implant. If that is not possible you should seek a board certified plastic surgeon with experience in chin and facial implants. Secondary deformity after implant removal occasionally may need additional correction rather then just implant removal. Good luck.

If possible, go back to your original surgeon and Have the chin implant removed. If not, seek out facial plastic surgeon who has done lots of Chin implants for best results. Make sure that you want to either simply take it out, or put it in the proper anatomical location if it's crooked. This can be done under local anesthesia as an outpatient procedure
