Hello, I have Trigeminal neuralgia and occipital neuralgia with TMJDD. I am only 40. I already need a facelift, neck lift necklift and lower bleph. I am concerned about any LASTING pain. Right now I'm am pretty controlled controlled without medicines but I just avoid touching that side. I don't don't want to lose any facial function or be incapacitated with severe pain either. Can you advise me about what I could reasonably expect as well as what to advocate for?

Obviously pain and pain syndromes like yours are not well-known and studied in medicine. Therefore, I don't think anyone can definitively say whether you will get permanent long-lasting pain after any of these procedures related to your conditions. I can say that I have operated on people with those neuralgias before and as long as we kept them on their standard medications, they were absolutely fine and had no exacerbation of symptoms. However, that is completely anecdotal.

With trigeminal and occipital neuralgia, I would not approach this as a routine facelift clearance question. Most facelift patients have temporary tightness, swelling, bruising, and numbness, but your baseline nerve sensitivity means a flare of neuropathic pain or more prolonged discomfort is a real concern. The exact pain response cannot be predicted online.
Before surgery, I would ask for a coordinated plan between your plastic surgeon, neurologist or pain specialist, and anesthesia team. Advocate for a medication plan if neuralgia flares, a clear discussion of facial nerve risk, and whether staging or modifying the procedure would reduce stress on your system. If you have uncontrolled severe pain, new facial weakness, vision changes, or new neurologic symptoms now or after surgery, seek urgent medical evaluation rather than waiting.

This is an important question, and in my opinion it deserves a very cautious answer.

With a history of trigeminal neuralgia and occipital neuralgia, I would tread very lightly with an elective procedure such as a facelift, neck lift, or lower blepharoplasty. Even though you may certainly have facial aging, tissue descent, and early changes that could be improved surgically, those aesthetic concerns have to be weighed against the possibility of disturbing a pain condition that is presently fairly well controlled.

The central issue is not simply what the average facelift patient can expect in terms of pain. It is whether surgery in someone with pre-existing neuropathic pain could trigger a flare, worsen sensitivity, or upset the balance you currently have. In a patient who has achieved reasonable control by avoiding certain triggers and not needing regular medication, that existing stability has real value.

In my experience, this type of situation should be approached thoughtfully and conservatively. A facelift does not treat trigeminal neuralgia or occipital neuralgia, and while many patients recover from facelift surgery without lasting pain, you are not really in the routine category from a nerve standpoint. The concern would be less about ordinary postoperative soreness and more about provoking a neuropathic pain response that may be difficult to predict or reverse.

For that reason, I do think this should be discussed carefully not only with your plastic surgeon, but also with the neurologist or physician helping manage your neuralgia. The question is not just “Can this be done?” but “Should this be done, given how well things are currently controlled?”

Sometimes the best answer is not to operate, at least not now. Avoiding an unnecessary disturbance in the harmony you currently have with your neuralgia may be the wisest course, even if there is a visible aesthetic benefit to surgery on paper. Elective facial rejuvenation should improve quality of life, not risk destabilizing it.

If you do pursue consultations, I would frame the conversation around risk tolerance, likelihood of nerve irritation, and whether the potential cosmetic improvement is truly worth the chance of awakening a more severe pain pattern.

There are some types of treatments that can reduce the likelihood of chronic pain in the context of personal situations already associated with this type of problems. However, no surgeon can match the level of expertise of a pain specialist, and the place to ask these questions is primarily with the pain specialist and then to expand regarding specific scenarios  with the surgeons.
