66 yrs old, 5'10", 145 ponds. I've heard about a new technick. Fat grafting into upper lids instead of cutting extra skin. It makes not hollow but younger-looking eyes. Your opinion?
Answer: Is Fat Grafting to the Upper Eyelids a Good Idea? I was the Course Director of our Academy’s fat grafting course for almost a decade. Most of my course was discussing the complications of fat grafting and teaching doctors what not to do. With that being said, I would strongly urge any patient who is considering upper eyelid fat graft to consider the risks because irregularities, asymmetries, and functional issues (with opening the eye and blink) can be much more of an issue in this region than other areas of the face. I typically like to start with fillers to see how patients like their appearance and then decide to move to something more permanent.

Answer: Fat Grafting to Upper Eyelids Fat grafting to the upper eyelids is an excellent treatment for eyelids with a volume deficit. It is exceptionally useful, and the most common patient in whom I do it, in someone who has had an upper blepharoplasty with fat removal, something I think should almost never be done. It does require exceptional skill since the area is more dangerous because of the larger vasculature and usually requires more than one session since there is not a lot of space to put the fat where it can be revascularized. Whether it would be appropriate for you or not would require an examination or, at least, good pictures to determined your amount of volume deficit. Make sure you see a plastic surgeon who is experienced in both fat grafting and facial and eyelid rejuvenation.
